Firstly, yeah the main part of a fetish is the fantasy aspect, wich I guess has some very few exceptions. Like... I Dunno: feet; it's hard to imagine this being harmful to two consenting individuals.

When a person makes it famous, like that one serial killer who captured, r*ped and took photos of their feet's victims, that's one person. Does not mean that every person with the same, or smiliar qualities of that person will have similiar or equal behavior.
There was a research, or more like prof, that people who abuse and/or dispose of their pet have an extremly high chance of being psychopaths. But that is a much more direct line of thought, due to complete negligence for other, or lack of complete lack of compassion (Psychopathy).
With that said, I think feet are like one of most famous (as in most people have it) fetishes. Trough admitance of course, but who knows...

I understand your concern with pedophilia and everything revolving around it, but I cannot agree with your philosophy that every one who draws underage art (weather by a year/less or a month less than 18, and weather the artist even knows about it) would be classified as a pedophile, Since the criminal definition is when it afftects actuall children and anyone under the age of 18 mentally or phisically. I can understand it being a red flag for that person being in that "danger zone" but that doesn't mean the person has completly trown away their moral values. My point is drawing "this type" of art doesn't mean a direct correlation to a person wanting to actually hurt children in real life.
By the way sorry for my bad english, not a native speaker. And I would like a to have a personal chat with you to understand your beliefs on this whole kind of stuff. A civilized conversation accompanied by a cup of tea
